WOODS CROSS, Utah (AP) _ Newly released surveillance video shows the moment a fiery gas explosion ripped through a Utah refinery
earlier this month.

The U.S. Chemical Safety Board on Tuesday released the video and
its preliminary findings of what led to the Nov. 4 blast at the
Silver Eagle Refinery in Woods Cross. The blast damaged 10 homes
but caused no serious injuries.

Company officials last week heeded a recommendation from
investigators to shut down the refinery until it's deemed safe to
reopen.

Investigators haven't made a final determination on what caused
the blast. Preliminary findings indicate that the explosion
happened when a 10-inch pipe failed catastrophically.
